Next Tuesday, June 28th, from 3-5 pm, FHN will be giving sports physicals at Orangeville.
Cost is $25.00 made to FHN, and no appointment is necessary. However, students must complete & have a parent/guardian sign the IHSA exam forms and FHN Consent for Sports Physical form prior to the exam. Forms are below, and will also be available at the school.
Remember, the sports physical meets IHSA eligibility requirements for all sports, but it does NOT meet the requirement for a 6th or 9th grade physical or for new students from out-of-state. (However, those full physicals will be accepted for for sports requirements.)
If your student will be getting the sports physical he/she should enter at the elementary end of the school. If you cannot make it to the Orangeville site, FHN is giving sports physicals at many area schools. See their website: https://www.fhn.org/sports-physicals-area.asp
